HCA: Revised Orthodontic Information form (HCA 13-666), eff 3/1/2022

The Health Care Authority (HCA) recently sent the following alert to dental Medicaid providers.

Attention Orthodontic providers:

The Health Care Authority (HCA) revised the Orthodontic Information form
HCA 13-666 (3/22). Providers must download the new form from the HCA Forms and Publications webpage. The revised form will be available on the Forms and Publications webpage on February 1, 2022. Orthodontic authorization requests must be submitted using the most current form on and after March 1, 2022, or they will be rejected.

Medicaid Transition to Managed Care Cancelled

The 2019 Legislature has directed the Washington State Health Care Authority (HCA) to continue to administer the dental Medicaid program through fee-for-service. The HCA may not proceed with a carved-out or carved-in managed care dental option at this time.
