Citizen of the Year Award
Each year the WSDA requests nominations for its most prestigious award, the Citizen of the Year. The WSDA is looking for those members who have displayed exemplary service to the community through civic/charitable activities. This may include service with organizations(non-organized dentistry), schools, non-salaried government positions or church sponsored community activities. Volunteer work does not have to be dental related. Nominators will ensure that any volunteer work is not associated with any position for which the nominee receives any compensation directly or indirectly.
The award is presented at the Citizen of the Year banquet held during the annual House of Delegates meeting in September. This year the banquet will be held on September 20th in Lake Chelan.
Nominations may be made by an individual member of the WSDA, a component dental society, or staff of a WSDA member dentist. The deadline for nominations is May 24.
To submit your nomination, please complete the form below and email additional documentation such as articles, newspaper clippings, and supporting letters from organizations that illustrate the nominee's activities to Amanda Tran at firstname.lastname@example.org. Previous nominations are kept on file for three years and may be resubmitted. Please note previously submitted nominations in the form below and include any new information on the candidate.
All nominations will be reviewed by the Task Force on Recognition of the WSDA. Candidates will be notified when a decision has been finalized.
