All Delta member dentists should have received their WDS/Delta proxy voting credentials to their email on Monday from the following:
- Sender: WDS Proxy Agent (wdsagent@proxy-agent.com)
- Subject: Your Electronic WDS/Delta Proxy
The email sent from the WDS Proxy Agent provides you with your Electronic Voting Number and allows you to submit your proxy quickly and easily online at www.cesvote.com. There is no need to mail in the hard copy of your proxy if you submit your proxy online.
Both the Washington State Dental Association and Delta Dental of Washington encourage you to submit your proxy now to approve amendments to the WDS/Delta Bylaws and Articles of Incorporation. These amendments are part of the proposed settlement of the lawsuit between WSDA and Delta. The settlement is contingent on Delta member dentists' approval of these Bylaws amendments.
Even if you intend to participate in the virtual meeting on Nov. 13, please submit your proxy online today in case you cannot attend. Attending the virtual meeting will supersede your proxy and you will be able to vote yourself on Nov. 13.
Instructions to Approve Proposed Amendments
Step 1: Check a box to assign your proxy to one of the three individuals listed below (all support the proposed amendments).
Step 2: Check the box FOR the election of Dr. Eve M. Rutherford, DDS.
Step 3: Check the box FOR APPROVAL of the proposed amendment to the Articles of Incorporation.
Step 4: Check the box FOR APPROVAL of the proposed amendments to the Bylaws.
