survey regarding how health care providers approach tobacco use with their clients and patients.
Responses to the survey will assist the DOH in the development of relevant client and provider resources. The survey is expected to take 5-10 minutes and you may respond anonymously unless you choose to provide your email address.
The response deadline is Friday, September 4 at 5 PM.
