WDIA provides group medical insurance quotes for employers and their employees from several leading insurance companies.

To qualify for a group medical plan you must have the following:

• At least two or more people on a plan,
• A tax identification number (social security number can be used if you are a sole proprietor),
• Most insurance companies require you to be in business at least two quarters, although some will allow you to be in business only one quarter. You must submit a copy of the last two quarters of your Employee Quarterly State Hour & Wage Tax Report (5208 A & B) for proof of hours worked.
