Submitting a WDS/Delta Proxy Vote

Washington dentists, by now, you should have received notice of two special meetings of WDS/Delta member dentists being held at 2 p.m. on Wednesday, September 6 at the Lynnwood Convention Center to consider a series of bylaw amendments that have been proposed by your fellow members.

WDS/Delta member dentists on the WSDA Board of Directors recommend that you vote in SUPPORT of these proposed bylaw amendments. You can do this in one of two ways: in person or by proxy. 

Are you an active WDS/Delta member but never received your proxy form, or did you already send your form to Delta but want to revoke it/change your vote? Click here for a blank form and instructions for how to complete it. (If you are submitting a replacement form, the form with the latest date will be the one that counts.)

Please see below for a few FAQs specifically regarding the proxy voting process.

1. I am planning to attend the special meetings. Do I need to complete a proxy form?

No. If you’ll attend in person, you don’t need to complete a proxy form and can vote in person. Please arrive by 1:00 p.m. for credentialing purposes. We would also like to make you aware that there will be a WSDA-sponsored pre-special meetings gathering at 11:30 a.m. in room 1DEF at the Lynnwood Convention Center. Lunch will be provided.

2. I cannot attend the special meetings and still have my proxy form. What should I do?

It is vital that you assign your proxy vote to another member dentist attending the event. Mailing your proxy vote to Delta Dental gives your voting rights to the Delta Board of Directors. 3. I didn’t receive a proxy form from Delta, or I threw my proxy form away. How do I obtain one?

If you are a WDS/Delta member dentist and don’t have a proxy form, please click here to find a blank proxy form along with instructions for how to complete it. If you are using this blank form, please make sure to include your printed name and license number on the form.

4. I have already submitted my proxy form to Delta. Can I revoke that proxy and/or change my vote?

If you already sent your proxy form to Delta and decide you want to revoke it, you may do so by completing a new form and assigning it to another member attending the meetings in person. Please make sure to include your printed name, license number, signature, and a clear date to validate this form and make clear it is meant to replace your previous vote. The form you submit with the latest date will be the one that counts.
